• You will see a “For Providers” listed at the top of the page; click on it.
•  After clicking the “For Providers” option, click “Become a Community Provider.” Then, complete the appropriate profile form on the website by clicking “Become a Community Provider” and submit it by mail/email/phone to apply to participate in Community Programs.
• Click Register Now.

EFT/ACH – Setting up EFT is a great way to receive payments quickly and easily. You have two options for EFT:

• To register to receive EFT through the Settlement Advocate for Community Health Choice only, visit https://view.echohealthinc.com/EFTERADirect/CommunityHealthChoice/index.html.
• Visit https://view.echohealthinc.com/EFTERA/efterainvitation.aspx to sign up to receive EFT from all payers who process payments on the Settlement Advocated platform.

Texas STAR Plan (Medicaid): Community Health Choice Medical offers a free medical plan for children and pregnant women for U.S. residents who cannot afford health insurance. This plan is for them.
